11. We have a one-time application fee of $250.00 to cover our initial costs of setting up your credentialing file.

If you would like to fill out your application on your computer, feel free to use our electronic version are required in a few places. Please mail the application to the CFPA address listed above with the initial credentialing fee.

It is recommended that you save a copy of the electronic version of the application to your local computer in case you need to come back to it later.

The work history portion of your CV MUST be in month/year format. Example: 03/1993 - 10/1999 Lakeland Helath Clinic. Any gaps need to be explained.
